Access Medical Clinic requires a signed Medical Record Release Form in order to obtain a copy of your medical records or to have your medical records sent to another health care provider. Patients may be charged a fee per page for a printed copy of their medical records.
Access Medical Clinic will not charge a fee if the patient requires this information for a claim for benefits under the Social Security Act or any other state or federal needs-based benefit program.
